I think there was a problem with b13 and USR modems not connecting well
that was supposedly solved by b17 according to the release notes.

This is a line from the B15 notes:

Interoperability problems with several V.34 modems have been fixed.

This is from B17:
* V.90 interoperability is improved.

* V.34 support in the "True Digital 56K Card" is improved.

same card? What happened when you put it back in 1st slot?

It would have to be problem with slot if it works in other slots.

Put a different card in slot 0 OR try orig card back in slot 0. Maybe it
wasnt fully seated.
