I have noticed this on our PRIs also. We're at 3.7.2, 5ESS, SWB, PRI. I
noticed this on previous ComOS versions including 3.7.1. We just upgraded
to 3.7.2 yesterday morning. If I dial the number again, it always seems to
go through. I'm betting it's a either a telco problem or our PBX fumbling
the outgoing call. I can't seem to reproduce the problem at will and I'm
not getting user reports of it so I'm not actively debugging it yet.
