45K bidirectional and 40K uploads were part of the original Lucent V.flex2
system.
To the best of my knowledge, these did not make it into K56flex. Sacrificed
to the gods of interoperability.
My understanding is Lucent is bringing them to the ITU-T as part of the
proposal process for the final standard. We may yet see them in some
future standard.
