The PMx knows the calling number before it knows the username. If the
session falls apart early, it may never learn the username.

On the other hand, on a CT1 on DMS switches, the box will never learn the
calling number (0 digits on DMS-100; 1 on DMS-10) so the username is needed
as an alternative.
