(PM) X2 won't authenticate

I've got a customer with a Sportster X2 modem. When he tries to connect the
modems answer and train and everything appears okay. Debug 0x51 shows
username and password authenticated and connection succeeded. However,
immediately I see

Received LCP_PROTOCOL_REJECT on port S5 of 18 bytes containing:
08 04 00 16 80 21 01 01 00 10 02 06 00 2D 0F 00 03 06 CD DA 4B 13

IP-Address [0x03], length: (6 bytes), [205.218.75.19]

And then a disconnect.

I can tell that it rejected IP protocol (I think) but I can't figure out
why.

The authentication never seems to complete. He never gets an IP address
assigned and no record hits the RADIUS detail file.

How can I start to figure out what's going on here. Is there a reference
somewhere for these codes. The PM Configuration Guide is pretty lame on
this subject.
