I can't speak for any other ISPs, but at mich.com we decided against charging
extra for 56K. For us the extra bandwidth consumed by the 56K customers (who
will probably get no more than 46K on average) doesn't justify the extra
expense of having to bill separately for the higher connect rate. In fact,
we even charge the same rate for 64K ISDN now, which 128K ISDN being double
the standard rate. The line costs and equipment costs are the same no matter
how they connect, and the bandwidth doesn't amount to that much money when
we calculated out how much we're paying for it from our NSP.
