I didn't remember this but it is in the release notes for 3.7:

In the "New Termination Codes for True Digital 56K Card" section it says:

Call Circuit Closed

This could be generated by any of the following reasons.
- Remote modem, on-hook condition.
- Three invalid login attempts.
- Sudden phone line termination.
- Remote modem powered off.

Normal LAPM Disconnect

This reason indicates a normal V.42 disconnect packet was received, and
is a normal call tear down.
