This just means that the RADIUS server got the same request more than
once. This can be caused by any number of things, but the most common
are:

a) network between PM and RADIUS server is saturated and packets are lost
and need to be resent.

b) system RADIUS server is running on is so loaded that it gets several
copies of the request in the queue before it gets a chance to process
them.
