(PM) Disconnects and failure to authenticate

I have a pm3 at a POP, dual CT1's, 24 trunks per. Folks
dialing in locally are having a hell of a time either a)
starting their connections, or b) staying logged on for
more than 5 minutes at a time. Most are Win95 users,
various modems, Supra 56k, PPI's, etc.

T's are configured ESF/B8ZS, switch is a 1A, but pretty
lightly propogated. I had a local CO tech on the line
for about 3 hours the other night, watching for disconnects
on their side of the wire, we didn't see much out of the
ordinary. Telco stated that they are sending us all seven
digits, which I have heard tell can screw the initial wink
start sequence, but I can't find much information to justify
changing this provisioning.

One thing that I do notice is the reason stated for last call
disconnect is "CIRCUIT DISCONNECT" (via sh mo). Does this
indicate that the PM3 received no terminate request, or that
the disconnect was telco related (maybe the same thing).

This one is really killing me, any help would be appreciated,
