Length from the CO isn't really an issue - these are digital lines which can
be cleanly repeated as needed. The "up to 2km" and similar values you've
gotten probably mean that the CSU/DSU itself can do a line build out which
can send the signal that far without a repeater in the circuit. T1/E1 lines
can run 10, 20, hundreds of miles with enough repeaters. (Up to a point I'm
sure, not sure.)

Yes, each E1 has its own CSU/DSU unit. The optional T1 WAN card has an
integrated CSU/DSU on it too but there isn't an E1 model (probably because
customers with E1's will usually use all 60 channels available thus all six
slots of modem cards - lucky them).
