What seems to work best is to send them an email telling them that if they
log on twice they get billed twice as much. Point out where this is
disallowed in the user agreement and if it continues bill them for
multiple accounts. Don't treat it as a negative, treat it as an
additional source of revenue. If they don't like it they will go
somewhere else and you should be pleased that they will be off hurting the
competition instead of you. Eventually you will get them back when the
competition goes out of business.
