Lost carrier means that the other end hung up the phone without going
through a formal connection shutdown procedure.

If at the end of the dial in session the user simply turns off his/her
PC then that causes a lost carrier disconnect. If the PC locks up for any
reason and the user needs to reboot then that causes a lost carrier
disconnect. If a roommate or child or spouse picks up the phone and
interferes with the connection then that causes a lost carrier disconnect.
If the user neglects to disable call waiting and another call comes in and
breaks the connection then that causes a lost carrier disconnect.

If Russia grows tired of NATO's expansionist policies and drops the big
one, resulting in nuclear winter then that causes a lost carrier
disconnect.
