The "Custom" notation probably refers to the switch type. There are
different "flavors" of 5ESS depending on the software they are running on
the switch. Pac Bell is probably running the custom software set. I think
that is the same as what I have down here. You might try setting your
switch type to NI-2. I think the command is:

set isdn-switch ni-2

Don't be afraid to try different settings. It can't hurt anything.

Also, you should check the lights on your smart jack at the telco demarc.
They may have the circuit in a loopback for testing. Sometimes they have
the circuit looped up somewhere waiting for authorization to release the
circuit.

If you don't have a loopback plug for your PRI cables, I would suggest
making one before you call Pac Bell and be ready to put it on the end of
your cable that goes into your line0 or line1 port on your PM3. That way
you can have them run a test on the line to be sure it is in good shape
before you put it in service.
