To follow-up on this, reflashed the 56K box, and it didn't help. I'd
orignally upgraded it to 3.7 with PMConsole 3.5.1, and I redid it with
3.5.3, per the installation instructions. It didn't help... it's still
not showing Connect-Info for anything above 33600, and doesn't show
it for 33600 and below more than half the time. I am running an old
version of Radius, which I can try upgrading, but it seems unlikely
that it would cause intermitant item loss in accounting packets. I've
never noticed it before.

I have also been noticing a problem where the "Host" column in "show ses"
will randomy show something like this on idle ports:
S4 - p1 Log/Net In IDLE 0 0
The "p1" may have another number instead of a 1, but always a p. Sometimes
it will affect almost all idle ports, with various different numbers, and
other times it will only affect one or two. Reseting the port always
clears it. Yesterday, I would get a message similar to "not a valid host
structure" when I reset some of the ports that were affected. Both units
have been reset since I noticed the problem, and it still occurs. Here's
a full "show ses" on a non-56K unit. The 56K unit is only showing the
S4 line above at the moment. Ports with users on them appear normal, and
have been removed from the display before. The last 3 ports I reset
manually while writing this.
